There's no hard limit, but 3–10 accounts tends to produce the best variety. More accounts means more tweets in the pool and more varied rounds.

No. Retweets are filtered out automatically - only original tweets from each account are used.
That account is silently skipped. As long as at least one account returns usable tweets, the game will start. If no tweets are found at all, the overlay shows an error message.
The game runs whenever the OBS browser source is active and the overlay URL is open. It does not require you to be live.
